The lumbar spine - five vertebrae sitting between your ribcage and pelvis - is the most mechanically loaded segment of the spine. The muscles that stabilise it are deep, powerful, and when they fail, the pain they produce is distinct from anything higher up the back.

The primary muscles driving lumbar pain — and why they are hard to treat:

  • Quadratus lumborum (QL):runs from the pelvis to the lowest rib, flanking the lumbar spine. The single most common overlooked source of deep lumbar pain.

  • Iliocostalis lumborum:the outer erector spinae layer connecting the pelvis to the ribcage. Overloaded in physical work and extended driving.

  • Iliolumbar ligament area:the transition zone between the lowest lumbar vertebra and the pelvis — a common site of deep, poorly localised aching.

  • Multifidus:deep segmental stabilisers that run along each lumbar vertebra. When they atrophy or spasm, they compress the disc space directly.

Extended driving and vibration exposure

Trades, painting, shelf stacking, and any work involving repeated overhead movement loads the supraspinatus and infraspinatus beyond their recovery capacity. The result is a rotator cuff that stays in a state of chronic low-grade inflammation and tension.

Leg length discrepancy and pelvic tilt

A functional or structural difference in leg length causes the quadratus lumborum on the shorter side to chronically hike the hip — creating an asymmetrical load on the lumbar spine that compounds over time into significant unilateral lumbar pain.

Inadequate recovery between physical demands

When physical work resumes before the lumbar muscles have fully recovered, each subsequent loading cycle adds to an accumulating tension debt. Without deliberate treatment, this debt eventually produces a baseline of constant lumbar aching that does not resolve with rest alone.
